Download the exact form and tax-year template from the IRIS Taxpayer Portal. Do not build a “close enough” CSV from memory.
Most preventable IRIS CSV problems happen before the upload: spreadsheet software changes dates or long numbers, users alter the IRS header row, or the wrong tax-year template is reused.

Publication 5717 says the uploaded column count must match the IRS template. Do not insert notes, helper columns or extra header rows.
Use IRS-permitted TIN types, Y/N checkbox values, two-letter state abbreviations and required country codes.
Dates should be in the required format, and long identifiers must not be saved in scientific notation.
The file you upload is the exported CSV—not the workbook view. Validate the final CSV after export.
Spreadsheet software can convert long numbers such as account or identifier values to exponential notation. Store them as text before export.
Publication 5717 says currency values should not include dollar signs or commas and may have no more than two digits after the decimal.
Use Y or N where the IRS template defines checkboxes; do not use TRUE/FALSE, X, Yes/No or 1/0 unless the current template says otherwise.
The Portal upload workflow permits one tax year and one form type per CSV file.
Split the file before upload. Publication 5717 (Rev. 9-2026) allows a maximum of 250 records per CSV file.
Do not change the IRS template structure. Map your source data into the template in the order provided.
Yes. Excel can hide encoding, quoting, delimiter, date and scientific-notation problems. Validate the saved CSV itself.
